Please complete the following form if you would like to buy an FIS digital credential from Exostar. Exostar offers three options with our service, so please be sure to select the one that best fits your needs. (For more information on FIS, read the Service Description.) The three options are:

- Includes three (3) certificates (Identity, Digital Signature, and Encryption)
- Valid for three (3) years from purchase date
- Includes 3rd party identity proofing in accordance with CertiPath policy
- Includes Key Escrow and Recovery options for the encryption certificate

| Medium Level of Assurance - Hardware* |
- Includes three (3) certificates (Identity, Digital Signature, and Encryption)
- Valid for three (3) years from purchase date
- Includes 3rd party identity proofing in accordance with CertiPath policy
- Includes Key Escrow and Recovery options for the encryption certificate
- Includes Department of Defense (DoD) interoperability
- Does not include Hardware token cost
